What We Offer
Support That Meets You Where You Are
Whether you are navigating a recent change, planning ahead, or looking for practical support in daily life, our services are designed to help you understand the situation, make informed decisions, and prepare for what comes next.
Care Management & Advocacy
Professional clinical guidance, health navigation, and dedicated family advocacy.
Care Management at Well & Able is a direct partnership for families navigating complex healthcare decisions, care transitions, aging needs, or special education accommodation hurdles. We evaluate the full situation, help identify clinical priorities, explore options, and build a clear path forward alongside your family.
How We Support Your Family:
- Medical & Doctor Visit Navigation: In-person or virtual appointment support, asking key clinical questions, taking detailed notes, and translating complex doctor instructions.
- Chart Oversight & Portal Monitoring: Ongoing review of medical histories, physician notes, and MyChart portals to keep clinical details from falling through the cracks.
- School & IEP/504 Advocacy: Reviewing educational evaluations, drafting clinical accommodation requests, and attending school meetings to ensure proper support.
- Care Transitions & State Resources: Step-by-step guidance through hospital discharges, facility options, and navigating North Carolina waiver programs (CAP/C, 1915(i)).
Seeing the Whole Picture
Medical care, school accommodations, specialist visits, daily routines, and long-term care planning all influence one another. When providers only see their specific piece of the puzzle, critical details get missed. We connect those moving parts so your family's decisions are grounded in the entire picture.
Consultation & Action Audits
Focused, virtual guidance when you need immediate direction, record reviews, or an actionable plan.
Not every family needs ongoing monthly care management. Sometimes you need targeted expertise to help you understand a complex situation, audit official records, and prepare for an upcoming meeting. Our consultations provide direct access to professional nursing guidance on a project basis—wherever you are located.
What We Offer:
- Virtual Chart & Medication Audits: A comprehensive nursing review of medical histories, physician orders, and medication lists, culminating in a clear Master Action Plan and video debrief.
- IEP & 504 Strategy Audits: In-depth review of psychological, educational, and medical files with a strategy prep session before your school board or IEP meeting.
- Focused Strategy Sessions: One-on-one virtual consultations to problem-solve immediate care hurdles, evaluate living or care options, or resolve complex family dilemmas.
- Long-Distance Caregiver Guidance: Providing professional perspective for out-of-state families who need help mapping out local support networks.
Frequently Asked Questions
Here are answers to common questions about our services and scheduling.
A Care Manager is a guide and trusted partner who helps individuals and families navigate healthcare, clinical choices, and care transitions with greater clarity, confidence, and support. We assess needs, evaluate options, create actionable plans, and coordinate care. Our role is to empower you to make informed decisions that align with your family's goals, preferences, and values—you always remain in control.
No. While many families first reach out during a hospitalization, a new diagnosis, or a sudden care shift, some of our most valuable work happens before a crisis occurs. Planning early helps preserve choices, clarify preferences, strengthen support systems, and improve readiness for future changes.
Families typically contact us for two reasons: some are facing an immediate challenge, such as a difficult diagnosis, specialist coordination, caregiver strain, or a major life transition. Others want to better understand the road ahead before decisions become urgent. In both situations, the goal is identical: gain clarity, preserve choices, and move forward with confidence.
Resources alone rarely solve complex care challenges. The real work is knowing which options fit a specific person's goals, budget, health situation, and family dynamic. We combine licensed clinical oversight with dedicated family advocacy to evaluate choices and implement practical plans that work in real life.
Earlier than most people think. Many community programs, care settings, and specialized support resources involve eligibility evaluations, paperwork, or waiting periods. Planning ahead does not guarantee a specific outcome, but it significantly expands your choices if circumstances change.
Many families contact us simply knowing that something needs attention, but unsure where to start. Our role is to help evaluate your situation, identify clinical priorities, explain available options, and determine what level of guidance makes sense. You do not need to have everything figured out before reaching out.
Yes. Advocacy is a central part of our work. Under HIPAA guidelines, patients have the legal right to designate advocates to attend medical visits. We can attend appointments in-person or virtually to ask critical clinical questions, take detailed notes, translate instructions, and keep family members updated.
No. We do not provide hands-on personal care services such as bathing, dressing, or physical transfers. Our practice focuses strictly on professional nurse care management, clinical oversight, doctor appointment navigation, record audits, and family coordination.
We provide in-person Care Management and appointment support across our local service area in North Carolina. Virtual consultations, chart audits, and strategy sessions are available to families nationwide.
